Respect in Sport - For Hockey Canada Parents
To be completed if your child is new to minor hockey, only one of the parents must complete this program.

Registration for the 2024-25 Season
If you have a player that has recently moved to our association, you will need to send an emailed "Player Transfer Request". Please include: your name, player's full name, player's DOB, player's HCR #, and last known association to FRMHA President - Kevin Lafreniere.

Hockey Canada Medical Information Sheet
You will need to complete one (1) PER CHILD registered within the FRMHA.

FRMHA Photo/Media Release Form
You will need to complete the form once PER FAMILY.

Code of Conduct
Please review for yourself, and your child(ren), as it applies under, "Players' Code", "Parent/Guardian's Code", & All Members' Code."
