Respect In Sport - For Hockey Canada Leaders
You will need your HCR # from this once completed to link yourself to your Spordle account listed below.

Vulnerable Sector Check Information:
VSC Request Letter
Member: Northern Ontario Hockey Association
Association/Club: French River Minor Hockey Association
You will then be required to submit this online or in-person to your local OPP or otherwise
Online Police Record Checks
Online link to OPP to apply for VSC, you are applying as a volunteer there will be NO COST.

OHF Screening Submission Portal
You must upload your completed VSC once it has been sent to you via .pdf using the portal above.


Once you have a Spordle account, you will need to complete the following clinic:

Important Information:
Hockey Trainer Code of Ethics & Hockey Trainer's Responsibilities
It is STRONGLY recommended that you keep a printed copy of each in your designated team's Trainer binder.

Emergency Action Plan (EAP)
It is MANDATORY for you to keep an accurate record of each arena your team attends, please keep within your designated team's Trainer binder.

Player/Team Injury Log
It is MANDATORY for you to keep a minimum of one (1) sheet in your designated team's Trainer binder.

Hockey Canada Injury Report
It is MANDATORY for you to keep a minimum of three (3) sheets in your designated team's Trainer binder.

Hockey Trainer Return to Play Form
It is MANDATORY for you to keep a minimum of one (1) sheet in your designated team's Trainer binder.
  
Hockey Canada Medical Information Sheet
It is MANDATORY for you to keep a minimum of two (2) blank sheets, as well as the completed forms from your designated team's players, and coaching staff (including yourself).
